The Rehabilitation Institute is an approved provider for most major insurance carriers and health plans.  To obtain pre-certification please use TAX ID #593140669.

If you would like us to assist in obtaining insurance authorization, please fax your physician order form and any pertinent information to 352.253.3809. Patients should bring their driver's license and insurance card to their scheduled appointment.

As a convenience to you, we will file your claims directly with your insurance company and will assist you in obtaining the maximum payment for a given procedure.  The Rehabilitation Institute is a Medicare approved facility and accepts assignment on all Medicare claims.  Any balance not paid by Medicare is forwarded to your secondary insurance company, where the balance is usually paid in full.

We also participate in many managed care plans (listed below) and accept payment in full for covered services of plans that do not have co-insurance or deductible requirements.

You are responsible for any co-payment, deductible, or non-covered services.  If necessary, you will be notified of any payment you need to bring with you to your appointment.  Ultimately, it is the patient's responsibility for any unpaid balance.  Your understanding and support is greatly appreciated.
